Abstract:The choice of controls in clinical trials of new antileprosy drugs involves problems which may be best solved, not by using individual controls, but by employing as a standard for comparison the average improvement in bacterial index made by large numbers of patients during standard DDS treatment. Such a standard makes it possible to compare one new drug directly with another. The procedure is illustrated in the case of three new antileprosy drugs, (a) Ciba 1906, (b) diaminodiphenyl sulphoxide (DDSO), and (c) I. C. I. Compound 15,688, or diethyldithiolisophthalate (ETIP). Ciba 1906, which has now been studied for 3-1/2 years, is an active and valuable drug which nevertheless makes its greatest achievements during the first six months of treatment, when it is usually decidedly superior to DDS. THereafter its activity approximates that of DDS. DDSO is slower but very steady in action, and has advantages and disadvantages compared with DDS. Compound 15,688 has a remarkable effect in some patients, producing a phenomenal change in the bacterial index and in the morphology of bacilli for a very short time, after which drug resistance develops. By the judicious use of these drugs during their period of optimal activity, associated where appropriate with DDS, it may be possible to attain new levels of effectiveness in the chemotherapy of leprosy. Abstract:The effects of various combinations of small doses of 4,4'-diaminodiphenyl sulfone (DDS), streptomycin and isoniazid (isonicotinic acid hydrazide) have been studied in intraperitoneally-infected mouse leprosy. The suppressive effects of various combinations of these drugs were definitely superior to those obtained with the individual drugs alone. The order of activity of the drugs and combinations used, based on the decreasing total indices of chemotherapeutic effectiveness, may be expressed as follows: (1) streptomycin, isoniazid, and DDS combined; (2) isoniazid and DDS; (3) streptomycin and isoniazid; (4) streptomycin and DDS; (5) streptomycin alone; (6) isoniazid; (7) DDS. Abstract:Studies have been made of the therapeutic effects on murine leprosy of several drugs and antibiotics, employing the intraperitoneally-infected mopuse. The following results were obtained: Amithiozone (TB1/698) seemed to cause some stimulation of the infection. PAS (p-aminosalicylic acid), B283 (a phenazine pigment), chlortetracycline (Aureomycin), oxytetracycline (Terramycin), Chloramphenicol (Chloromycetin), penicillin, erythromycin (Ilotycin), and three derivatives of diphenylthiourea, i. e., SU-1795, Su-1906 and SU-2358, revealed no significant activity. Title:C-reative protein in serum of patients with leprosy.

Abstract:The levels of C-reative protein (CRP) in the sera of 100 patients with leprosy have been determined. This abnormal protein was found in 79 per cent of 47 cases of the lepromatous leprosy classed as "active" (i. e., not arrested), 30 per cent of 41 cases of apparently arrested lepromatous leprosy; and 58 per cent of 12 tuberculoid cases. Possible explanations for the absence of CRP in some of the active cases, and its presence in some of the apparently arrested cases, have been offered. The importance of long-term studies, with correlation of clinical course and serial determinations of CRP, is stressed. The presence of CRP in 19 of 24 cases of leprosy with secondary amyloidosis has been described. The relationship of CRP to amyloidosis is a yet unknown, but the present findings would suggest that the problem merits further study. (AU).

Abstract:Six tuberculoid cases were treated with TB-1, and all resolved within three and aone-half years. Two illustrative cases are described. Thirty-seven lepromatous cases were treated with TB-1 for three and one-half years, with or without adjuvant sulfones. Only 4 cases became negative. Three illustrative cases that remained strongly positive are described. Thiosemicarbazone appears to be of value in tuberculoid leprosy. Its action is weak in lepromatous leprosy, and in combination with a sulfone the two drugs seem to be antagonistic. (AU).

Abstract:A brief review of the pharmacological actions of nicotinamide and allied compounds in relation to the mycobacterial infections has been presented. The activity of nicotinamide and pyrazinamide (Aldinamide) in mouse leprosy has been studied, employing the intraperitoneal route for infection. Comparative studies of the activity of these compounds with that of three known effective drugs- isoniazid, streptomycin and DDS - were made in animals treated immediately after inoculation or after delays of one or two months. The duration of the experiments was three months. Both nicotinamide and pyrazinamide were found to be highly effective in the suppression of the leprous infection. Nicotinamide was found to have a degree of activity similar to that of pyrazinamide and of isoniazid, and superior to that of streptomycin; DDS was the least active. All these five compounds were most effective when the treatment was delayed for one or two months. Only nicotinamide, pyrazinamide and isoniazid possessed any significant activity when the treatment was delayed for two months and then carried out for only one months. (AU).

Title:Mortality of persons with leprosy prior to sulfone therapy, Cordova and Talisay, Cebu, Philippines.

Abstract:1- A study is reported of the mortality of leprosy patients in Cordova and Talisay, Cebu Province, Philippines. The data are regarded as exceptionally accurate, having been derived from records of households inncluded in three surveys of Cordova (1933, 1941 and 1948) and two of Talisay (1936-37 and 1950-51), all made by the staff of the Leonard Wood Memorial in cooperation with the Department of Health of the Philippines. For Cordova, there are included 15 years of experience, 1933 to 1948, and for Talisay, 14 years, 1936-37 to 1950-51. Most of the bacteriologically positive patients had been treated for some years at the Eversley Childs Sanitarium. Sulfones were rarely used in therapy at that institution before 1947, and only for a few selected patients until 1951. 2- Standardized death rates have been computed for persons 15 years of age and over for each community and for both combined, by sex, for the total population, and for persons suffering from lepromatous or from nonlepromatous (tuberculoid or indeterminate) leprosy. For Cordova the second survey fortunately divided the experience at a critical point, and rates were obtained for the period 1933 to 1941, which are compared with those of the World War II period and the years immediately following, 1941 to 1948. 3- During the intervals between the initial and last surveys, for both communities combined, the average annual mortality for persons with lepromatous leprosy was 5.1 times that of the general population; for male patients the ratio was 5.1:1.0, and for females 4.4:1.0. Excess mortality over that of the general population was 71.7 per 1000 per year for patients of both sexes - 78.2 for male patients, and 53.3 for female. A larger experience is necessary to determine whether the observed greater excess mortality in males is a genuine characteristics or a chance variation. Patients suffering from the nonlepromatous forms did not have appreciably higher death rates than the general population. 4- Among lepromatous patients of both communities, the ratio of actual deaths to those expected at age specific rates prevailing in the general population. 4- Among lepromatous patients of both communities, the ratio of actual deaths to those expected at age specific rates prevailing in the general population was 4.0:1.0 for patients 15 to 29 years of age, 7.2:1.0 for those 30 to 49 years old, and 2.5:1.0 for those 50 years and over...(AU).

Abstract:Leprosy, a chronic infectious disease caused by Mycobacterium leprae, was identified by G. H. A. Hansen in 1873. The different clinical presentations of the disease are determined by the quality of the host immune response. The bacteria have affinity for the peripheral nerves and are likely the cause of neuropathy, a cardinal manifestation of the disease. WHO recommends a protocol of multidrug therapy (MDT), which effectively controls the disease, hence contributing to the global elimination program. Early detection of leprosy and treatment by MDT are the most important steps in preventing deformity and disability. (AU).

Abstract:Erythema nodosum leprosum (ENL) is a well-known serious complication affecting 10 per cent of lepromatous multibacillary leprosy patients. In the chronic form, its morbidity may be considerable. Thalidomide and systemic steroids are the two current effective drugs for the management of ENL. However, their use in endemic countries is often difficult and hazardous, and a search for new therapies is needed. We report our experience on the effects of pentoxifylline, a methylxanthine derivative, which has recently been suggested as a possible effective treatment for ENL attacks. (AU).

Abstract:The influence of concurrently rifampicin and clofazimine on the metabolism of 4,4'-diaminodiphenyl sulfone (dapsone, DDS) has been studied in 30 subjects on multidrug regimens for treatment of leprosy. Plasma and urinary levels of drugs were determined on days 2,8 and 15 after administration of the drug, while cretinine levels in urine were also determined to overcome the effect of diuresis. During concurrent administration of rifampicin the plasma levels of DDS gradually fell from day 2 to day 15 of rifampicin administration and the decrease was most significant on day 15. Clofazimine did not exert any such influence on DDS metabolism. A comparison of urine and plasma levels of DDS during the course of treatment. The findings suggest that during concurrent administration of DDS and rifampicin, the intake of DDS shoud be regular and uninterrupted.(AU).
